Organization can confirm it has appropriate 501(c)(3) status and will provide a valid W-9 to accompany the fund raiser request form and submit to BlazinMongolianBBQ@gmail.com. Requests without a valid W-9 attached will not be reviewed.

Organization understands that Blazin’ Mongolian BBQ will issue checks by using the information listed on the W-9. Blazin’ Mongolian BBQ will not issue or mail checks to any names or addresses not provided on W-9. Any checks lost in transition and resulting in a new check being cut will incur a $25 fee, which will be taken out of the amount raised.

Organization agrees to distribute only the flyers that Blazin’ Mongolian BBQ has approved to contacts and database at its own expense. Flyers may only be posted on Organization’s website and social media pages. Unauthorized posts to third party sites will result in a cancelation of the fundraiser.

Organization agrees not to create any additional materials with the use of the Blazin’ Mongolian BBQ name and/or logo and to not release information to the press without Blazin’ Mongolian BBQ’s approval. Violation of this agreement will result in cancelation of the fundraiser.

Organization agrees not to distribute flyers inside or near any Blazin’ Mongolian BBQ’s restaurant or to distribute flyers in public areas (e.g., on parked cars, in shopping centers, at coffee shops), and only to give flyers to actual supporters of the organization. Violation of this will result in cancelation of the fundraiser.
